急性脑梗死超选择性动脉溶栓与静脉溶栓的比较研究

目的：比较研究急性脑梗死患者应用超选择性动脉溶栓与静脉溶栓的临床效果和安全性。方法随机将本科收治的急性脑梗死患者76例分成对照组和试验组各38例，对照组采用尿激酶超选择性动脉溶栓治疗，试验组采用行静脉溶栓治疗。比较两组患者治疗后不同时间的NIHSS评分和治疗后30 d的总有效率。结果试验组治疗后30 min、12 h、1 d、3 d、7 d、14 d的NIHSS评分均低于对照组，两组比较差异有统计学意义（P〈0.05）；试验组治疗的总有效率为86.84%，对照组治疗的总有效率为71.05%，试验组高于对照组，两组比较差异有统计学意义（P〈0.05）。结论尿激酶超选择性动脉溶栓治疗效果优于静脉溶栓治疗，值得在急性脑梗死的治疗中广泛推广应用。

Comparison and research on supperselected intraaterial thrombolysis and venous thrombolysis in treating acute cerebral infarction

Objective To comparison and research the clinical effect and security of supperselected intraaterial thrombolysis and venous thrombolysis in treating acute cerebral infarction. Methods 76 patients with acute cerebral infarction in our department were randomLy divided into experimental group （38 case）and control group（38 case）. The supperselected intraaterial thrombolysis with urokinase were applied in experimental groups, the venous thrombolysis were applied in control group. Comparing the NIHSS score after their treatment in different time and the total effective rate after 30 days. Results The NIHSS score of the experimental group was lower than that of the control group after their treatment in 30min、12 h、1 d、3 d、7 d、14 d later, the difference between the two groups is significantly （P〈0.05）.The total effective rate of the experimental group was 86.84%,the total effective rate of the control group was 71.05%. the total effective rate of the experimental group was higher than that of the control group, the difference between the two groups is significantly （P〈0.05）. Conclusion The clinical effect of supperselected intraaterial thrombolysis with urokinase is better than that of venous thrombolysis. It is worthy of widely use in treating acute cerebral infarction.
