系统性红斑狼疮血液学异常的临床分析

采用回顾性方法分析系统性红斑狼疮（SLE）血液系统损害的特征,将445例确诊的SLE患者根据血液学异常分为WBC减少组、PLT减少组、贫血组及全血细胞减少组,血液学正常的SLE患者为对照组.血液学正常94例（21.1％）,血液学异常351例（78.9％）,其中贫血249例（56.0％）,白细胞减少123例（27.6％）,血小板减少60例（13.5％）,全血细胞减少38例（8.5％）.贫血组患者的肾损害发生率高于对照组（P＜0.01）;血液系统损害的SLE患者血C3水平更低（P＜0.05）,疾病活动指数（SLEDAI）更高（P＜0.01）.贫血是SLE最常见的血液系统损害表现之一,血液系统损害的SLE患者疾病活动明显,其血C3水平更低,SLEDAI评分更高.

Clinicalanalysesofhematologicalabnormalitiesinsystemiclupuserythematosus

Toexplorethecharacteristicsofhematologicalabnormalitiesinpatientswithsystemiclupuserythematosus（SLE）throughretrospectiveanalysis.Atotalof445SLEpatientswererecruitedanddividedinto5groupsoflowwhitebloodcell,immunethrombopenicpurpura,anemia,pancytopeniaandcontrolwithouthematologicalabnormalities.Amongthem,351（78.9％）hadhematologicalabnormalitiesand94（21.1％）withouthematologicalabnormities.Theprevalenceofanemia,leucocytopenia,thrombocytopeniaandpancytopeniawere56.0％（246/445）,27.6％（123/445）,13.5％（60/445）and8.5％（38/445）respectively.Theincidenceofrenaldamageinanemiagroupwassignificantlyhigherthanthatincontrolgroup（P＜0.01）.SLEpatientswithhematologicalabnormalitieshadlowercomplementC3levels（P＜0.05）andhighersystemiclupuserythematosusdiseaseactivityindex（SLEDAI）（P＜0.01）.AnemiaisthemostcommonfindingsinSLEpatientswithhematologicalabnormalitieswhohavelowercomplementC3levelsandhigherSLEDAI.
