胰岛素泵在妊娠合并糖尿病患者的应用及护理

目的探讨胰岛素泵在妊娠合并糖尿病患者的应用及护理。方法选取商丘市第一人民医院妊娠合并糖尿病患者54例,其中采用皮下注射胰岛素治疗的患者作为对照组26例,采用胰岛素泵治疗的患者作为观察组28例,分别比较两组的各项数据指标。结果所有患者治疗后的血糖水平得到了明显改观,两组患者在治疗后餐后2h血糖(6.8±2.3)mmol/L,(5.9±2.1)mmol/L;胰岛素用量(44.9±6.7)U/d,(38.4±5.0)U/d;糖化血红蛋白(6.3±1.1)%,(5.2±0.7)%;酮症酸中毒(5/26,1/28)、新生儿低血糖(6/26,1/28)、羊水过多(11/26,5/28)、巨大儿(4/26,1/28)和早产儿(9/26,3/28)方面差异显著P<0.05,有统计学意义,其余指标差异不显著。结论胰岛素泵在治疗妊娠合并糖尿病方面疗效显著且能减少母婴并发症的发生,采用有效的心理护理、置泵前护理和置泵后护理是一种非常有效的方法,值得临床广泛推广使用。

Application and Nursing of Insulin Pump in Pregnancy Associated with Diabetes

Objectives Study of application and nursing of insulin pump in pregnancy associated with diabetes.Methods We selected this courtyard of pregnancy associated with diabetes in 54 cases,where the subcutaneous injection of insulin in treatment of the patients as a control group of 26 cases,while we used insulin pump in the treatment of patients in observation group of 28 cases.We compared between the two groups of various data.Results All the patients after treatment of blood glucose level were markedly improved.Two groups of patients after treatment with 2h postprandial plasma glucose（6.8±2.3）m mol/L,（5.9±2.1）mmol/L,insulin（44.9±6.7）U/d,（38.4±5.0）U/d and glycosylated hemoglobin（6.3±1.1）%,（5.2±0.7）%,ketoacidosis（5/26,1/28）,neonatal hypoglycemia（6/26,1/28）,polydramnios（11/26,5/28）,macrosomia（4/26,1/28） and preterm infants（9/26,3/28） difference between（P0.05）,with statistical significance,the other indexes were not significant.Conclusions Insulin pump in the treatment of pregnancy complicated with diabetes mellitus curative effect and can reduce maternal complications,is a safe and effective method,is worth widely clinical use.
