摘    要:目的探讨并评价转铁蛋白(Tf)/血红蛋白(Hb)联合免疫法的临床应用价值,为消化道出血性疾病提供辅助诊断依据。方法用联合免疫法和化学法分别对112例消化道出血患者和65例阴性对照粪便标本进行检测。查阅病历资料,以内镜检测为金标准,探讨其灵敏度和特异性。结果化学法的特异性、敏感性、阴性预测值和阳性预测值分别是57.1%、96.9%、97.0%、56.8%,而联合免疫法分别为96.4%、98.5%、99.1%、94.1%,化学法检出限高于联合免疫法。结论联合免疫法是目前检测粪便隐血的较好方法,可有效提高上消化道出血及大肠肿瘤的阳性检出率。

Abstract:Objective To explore and evaluate the application value of fecal occult blood detected by combined transferrin/hemoglolin colloidal gold method,and to provide evidence for diagnosis of homorrhage of digestive tract.Methods The fecal samples from 112 patients with digestive tract bleeding and 65 healthy subjects were tested by combined immunoassay and chemical method.For all patients,the hemorrhage of digestive tract was diagnosed by endoscopy,a gold standard method.The sensitivity and specificity of combined immunoassay and chemical method were calculated respectively.Results The specificities sensitivities and predictive values of negative and positive were 57.1%,96.9%,97.0% and 56.8% in chemical method and were 96.4%,98.5%,99.1% and 94.1% in combined immunoassay.The detection limit of chemical method was higher than that of combined immunoassay.Conclusions Combined immunoassay could be a good method in detecting fecal occult blood.Combined immunoassay could improve significantly the positive detection rates of upper digestive tract bleeding and colorectal tumors.
