多西紫杉醇联合5-氟尿嘧啶和顺铂治疗晚期胃癌60例临床观察

目的观察多西紫杉醇联合5-氟尿嘧啶(5-FU)和顺铂方案治疗晚期胃癌的临床疗效和毒副反应。方法选择60例晚期胃癌患者。给予多西紫杉醇75mg/m2,第1天;顺铂20mg/m2,第2~6天;5-FU 375mg/m2,静脉泵持续点滴24h,第2~6天;1个周期为21d。治疗至少2个周期后评价疗效和毒副反应。结果全部60例患者完成治疗可评价疗效,完全缓解(CR)2例,部分缓解(PR)29例,稳定(SD)22例,进展(PD)7例,有效率(RR)为51.6%。中位进展时间是6.2个月,中位生存期是10.1个月,1年生存率31.5%。不良反应主要为胃肠道反应和骨髓抑制等。不良反应多为Ⅰ~Ⅱ度,耐受性良好。结论多西紫杉醇联合5-氟尿嘧啶、顺铂方案治疗晚期胃癌临床疗效较好,毒副反应可耐受,可以作为临床一线治疗方案。

Clinical Observation of Docetaxel Combined with 5-fluorouracil and Cisplatin in the Treatment of 60 Patients with Advanced Gastric Cancer

Objective To observe the efficiency and toxicity of docetaxel combined with 5-fluorouracil and cispatin regimen in patients with advanced gastric cancer.Methods 60 patients with advanced gastric cancer diagnosed pathologically were enrolled into the study.The patients received docetaxe 75mg/m2 on day1;cisplatin 20mg/m2 on day2 to 6,and 5-FU 375mg/m2 infused with an ambulatory pump on day 2 to 6;repeated every 21days at least courses.Results All patients were assessable for toxicity and response to treatment.Two complete responses and twentry-nine partial responses were observed.Stable disease was observed in twentry-two patients,with progressive disease in the other seven patients.overall response rate was 51.6%.Median time to progression and median overall survival were 6.2 and 10.1 months,respectively.The one year survival rate was 31.5%.The most common toxicities were myelosuppression and nausea/vomiting（Ⅰ-Ⅱ）,and well tolerated.Conclusion Docetaxel combined with 5-fluorouracil and cisplatin is a safe and effective regimen in the treatmentof advanced gastric cancer.The toxicity and side effects are tolerable.
