343例窒息新生儿肾功能动态分析

摘    要:目的探讨窒息对新生儿肾功能的影响。方法对343例窒息新生儿出生后一周内血尿素氮(BUN)和血肌酐(Scr)的动态检测结果进行分析。结果343例窒息新生儿中发生肾功能损害68例,发生率为19.83%。窒息组生后24~48h血BUN和Scr浓度均高于24h内,P0.01。窒息组24h内及24~48h血BUN和Scr浓度均高于对照组,P0.01。轻、重度窒息后肾功能损害发生率分别为19.87%、19.57%,差异无显著意义,P0.05。两组间肾功能恢复至正常水平的平均时间(轻度组4.6d,重度组6d)比较,各时间段Scr浓度比较,24h内血BUN浓度比较,肾功能损害患儿中低钠血症发生率比较,均无显著差异,P均0.05。结论窒息新生儿肾功能损害发生率高,发生率与窒息程度无关。对窒息新生儿应常规动态监测血BUN和Scr,可以及时发现肾功能损害。

Abstract:Objective To investigate the impact of asphyxia on neonatal renal function. Methods The blood urea nitrogen (BUN) and the blood creatinine(Scr) in 343 asphyxia] neonates within one week afer birth were detected. Results There renal function damage occurred to 68 cases (19.83%) .In asphyxial group .the levels of BUN and Scr 24~48h after birth were higher than than within 24h after birth (P〈0.01).The incidence of renal damage in slightly asphyxial group was 19.87% and in seriously group was 19.57%(P〉0.05). Conclusion The incidence of renal function damage in asphyxial neonates are higher and it does not relate to the degree of asphyxy.The levels of BUN and Scr in asphyxial neonates should be routinely dynamically monitored for renal function damage.
