分级步行锻炼对慢性心力衰竭患者心功能的影响

目的:探讨不同距离步行锻炼对慢性心力衰竭(CHF)病人心功能的影响。方法:164例CHF病人测定6WMT距离后,随机分为无锻炼组和按6WMT距离的1/4、2/4、3/4及4/4四个锻炼组,即1/4组、2/4组、3/4组和4/4组。锻炼组患者在其首次6WMT测试中完成步行同等距离的相应时间内完成步行锻炼,每天锻炼3~4次。比较各组2个月后6WMT距离和左室射血分数(LVEF)。结果:6MWT距离和LVEF于锻炼前后在3/4组有非常显著的差异(P=0.0002)。与其他组比较,不同心功能分级患者在3/4组6 MWT距离和LVEF增长百分比均明显升高(P<0.05)。结论:按3/4 6 MWT试验距离及相应时间进行锻炼更有益于慢性心衰患者心功能的改善。

Graded walk exercise on cardiac function of chronic heart failure patients

Objective:To investigate the effect of graded walk exercise on cardiac function in patients with chronic heart failure(CHF).Methods:By measuring the distance each patient covering in a 6-minute walk test(6MWT),164 patients with CHF were randomly assigned to group of non-exercise(NE group),one fourth of the distance(1/4 group),one half of the distance(2/4 group),three quarters of the distance(3/4 group) and full distance(4/4 group).All patients in the four exercise groups went through the program in accordance with the distance being covered in the given corresponding time of the first 6MWT,and the activity ranged from 3 to 4 times a day.The patients from the 5 groups received examination in distance of 6MWT and left ventricular ejection fraction(LVEF) after 2-month session of the program.Results:The covered distance in 6MWT and LVEF were found significantly different in 3/4 group as compared with pre-exercise(P=0.0002),and the increased percentage of distances and LVEF in 3/4 group was higher than that in other groups(P<0.05).Conclusion:The patients with CHF may benefit from the exercise by covering three quarters of the distance in a 6-minute walk test in the corresponding given time since it can lead to improving the cardiac function in this test.
