承德地区HBV基因型分布及临床意义的研究

目的研究承德地区HBV基因型分布及其与临床的相关性。方法收集本地区乙型肝炎表面抗原阳性的血清标本145例,采用FQ-PCR技术进行HBV DNA定量检测及HBV基因分型,并分析基因型与临床的相关性。结果所有阳性标本中B基因型12例(8.28%),C基因型107例(73.79%),BC混合型9例(6.21%),D基因型1例(0.7%),无法分型16例(11.03%)。同时发现基因型BC组在HBeAg阳性率,病毒载量及肝功能指标上均显著高于C组和B组(P<0.05),而C型又高于B型。结论 HBV基因型存在地区差异,承德地区以C型为主,并且C型对肝脏损害较B型严重;而BC混合型对肝脏的损害更为严重。

Distribution hepatitis B virus genotypes and its clinical significance in Chengde district

Objective To investigate the distribution of HBV genotypes and its clinical relevance in Chengde district.Methods 145 HBsAg positive serum samples were quantified in viral load and genotyped by FQ-PCR and analyzed the correlation between genotypes and clinical status.Results The cases of B genotype,C genotype,BC mixed genotype and D genotype were 12（8.28%）,107（73.79%）,9（6.21%）and 1（0.70%） respectively.There were still 16 cases of unknown genotype.HBeAg positive rate,viral load and indexes on liver function of BC genotype were significantly higher than that of C and B genotype and this suggested that BC mixed genotype was more dangerous than C and B（P0.05）;and compared to B,that was higher in C genotype.Conclusion Regional difference is found in HBV genotypes and C genotype is dominant in Chengde.Correlation exists between clinical hepatitis and genotypes,C genotype accounted for most of severe hepatitis and liver cancer patients,which suggested that patients with C genotype have worse prognosis,and BC the worst.
