MECT联合治疗伴兴奋攻击行为的难治性分裂症的对照研究

目的：比较氯氮平联合无抽搐电休克（MECT）、及单用氯氮平治疗伴兴奋攻击行为的难治性分裂症的疗效及安全性。方法：对60例伴兴奋攻击行为的难治性分裂症患者随机分为两组，A组氯氮平联合MECT、B组单用氯氮平，分别在治疗前及治疗后第2、4、6、8周末，进行PANSS评定，以PANSS的P4、P7、G8、G14四条目评定兴奋症状，以S1、S2、S3三条目评定攻击危险性症状，并在第8周末评定其疗效，以及用TESS评定不良反应，在治疗前及治疗后第4、8周检查脑电图。结果：兴奋攻击因子评分A组在治疗的第2周末，B组在治疗的第6周末均有明显下降；治疗结束时，联合治疗组总体疗效明显高于单用氯氮平组，A、B组间比较疗效无明显差异；两组不良反应无明显差异。结论：MECT联合氯氮平能快速、有效地控制难治性分裂症及其兴奋攻击行为。

The effect of clozapine combining MECT for the resistant schizophrenics with agitation and aggressive behavior

Objective:To compare with the effect and tolerate of different treating methods for the resistant schizophrenics with agitation and aggressive behavior. These different methods were clozapine combining MECT and clozapine alone. Methods:60 resistant schizophrenics with agitation and aggressive behavior were divided into two groups randomly. The patients of A group were treated with clozapine combining MECT. The patients of B group were treated with clozapine alone. The PANSS was used to rate the patients at the end of week second, fourth, sixth, eighth. The agitation symptoms were evaluated by P4,P7,G8,G14 in PANSS. The aggressive was evaluated by S1,S2,S3. We rated the side effects by TESS. The EEG was examined at the end of the week fourth and eighth.Results: The agitation and aggressive symptoms reduced significantly at the end of the week second in A group, the week sixth in B group. At the end of study, the improvement of combining groups was significantly higher than the control group and the side effects were not significantly difference. Conclusions: The clozapine combining MECT was effective for the resistant schizophrenics with agitation and aggressive behavior and can reduced those behaviors quickly.
