成纤维细胞生长因子-2介入治疗周围血管缺血性疾病实验研究

摘    要:目的研究局部动脉内灌注成纤维细胞生长因子-2(fibroblastgrowthfactor-2,FGF-2)对改善兔部分缺血的血管重建、血液灌流和肢体功能的作用。方法取成年雄性新西兰大白兔18只,外科结扎右股动脉各主要分支,制成右后肢部分缺血模型。每只兔子的左后肢作为非缺血对照。随机分成治疗组(n=9)和对照组(n=9)。动脉结扎术后即刻及术后8天、15天分别经动脉灌注FGF-210μg(4ml),对照组于相应时间给予同等体积生理盐水。在术后22天观察治疗组与对照组缺血区肌肉组织的血管断面数(VSC)、血管断面面积(VSS)、血管断面周长(VSP)状况,缺血肢体毛细血管再充盈时间以及缺血肢体的功能和营养状况。结果术后22天,FGF-2治疗组缺血肢的VSC、VSS及VSP值较对照组显著增高(P<0·01)。缺血肢体的毛细血管再充盈时间、新毛生长、功能及外观均接近正常;而对照组的肢体缺血性表现、毛细血管再充盈时间和肢体功能却无明显改善。结论经动脉内局部灌注FGF-2可明显促进缺血肢体的血管生长和血液灌流,从而改善其局部血液供应。

Abstract:Objective To evaluate the effect of intra-arterial infusion of fibroblast growth factor (FGF-2) on improving neovascularization, vascular perfusion and the function of partially ischemic limbs of rabbit. Methods Eighteen New Zealand male rabbits were selected. Partial ischemia model was induced by surgical ligation of the primary branches of right femoral arterial in each animal, and the left hind limb of each animal was served as a nonischemic control. Then, 18 rabbits were randomly assigned to two groups: intra-arterial (IA) infusion of FGF-2 ( n =9) and IA infusion of saline ( n =9). Infusion was separately performed in immediately after vessel ligation, 8th and 15th days post-surgery, 10 ug (4 ml) of FGF-2 per-time (or the same volume of saline) was used. The differences between two groups and between ischemic and nonischemic limbs of the same group were compared and evaluated by the following indexes: vessel section count (VSC), vessel section surface area (VSS) and vessel section perimeter (VSP) in the field of ischemic muscle tissues taken at 22nd day post-surgery; capillary refilling time of ischemic limbs; and functional and trophic changes of ischemic limbs. Results VSC, VSS and VSP of the IA-FGF-2 group were significantly increased than that of IA-Saline groups ( P <0.01). At 22nd day post-surgery, the capillary refilling time, new hair growth, the appearance and function of all ischemic limbs in IA-FGF-2 group were become approximately normal. However, in IA-saline group, the ischemic changes, capillary refilling time and the function of ischemic limbs were not improved significantly. Conclusion Intra-arterial infusion of FGF-2 may significantly promote neovascularization and vascular perfusion in ischemic limbs, and lead to improvement of its blood supply.
