主动脉球囊反搏治疗心肌梗死后无复流和慢血流临床研究

目的研究主动脉球囊反搏术(IABP)对急性心肌梗死患者行PCI术后出现无复流/慢血流的治疗作用。方法将2006年—2011年在我院因心肌梗死行急症PCI出现无复流和慢血流的患者随机分为应用IABP和不用IABP的两组(慢血流和无复流患者分别分为应用IABP组和未应用IABP组),比较两组的患者BNP、醛固酮(ALD)、血管紧张素2(AngⅡ)、左室射血分数(LVEF)等的改善情况,从而评价IABP对PCI术后无复流/慢血流的治疗作用。结果应用IABP的患者BNP、ALD、AngⅡ较未应用IABP的患者峰值提前,降速增快,差异有统计学意义(P<0.05),应用IABP的患者EF值较未应用IABP的患者升高,差异有统计学意义(P<0.05)。结论应用IABP对改善心肌梗死行PCI后无复流/慢血流患者的预后起到积极作用。

The Effect of IABP in No-Reflow and Slow-Flow After Primary PCI in AMI Patients

Objective To research the effect of intra-aortic balloon pumping(IABP)on no-reflow and slow-flow phenomenon in emergency percutaneous coronary intervention(PCI).Methods Clinical data of acute myocardial infarction patients after PCI with angiographic no-reflow/slow-flow phenomenon were retrospectively analyzed between January 2006 and December 2011.Patients were classified into IABP team and none-IABP team randomly.Observed differences of brain natriuretic peptide,vasoactive substances including aldosterone(ALD),angiotensin Ⅱ(ANGⅡ)in both groups on the days of 1,2,3,5,7,10 after the different interventions.In addition,left ventricular ejection fraction(LVEF) was evaluated after 10 days,3 months,6 months.Finally,statistics was token to analysis the data.Results The several vasoactive substances as well as LVEF of 10 days,3 months,6 months of IABP groups had significant difference from control groups(P<0.05).BNP targets of IABP groups compared with the control groups had significant differences(P>0.05).Conclusion IABP has effects on prognosis in AMI patients who performed PCI with angiographic no-reflow/slow-flow phenomenon.
