Orem自护理论应用于脑卒中后遗症辨证施护的效果分析

目的观察Orem自护理论应用于脑卒中后遗症患者辨证施护的效果。方法将120例脑卒中后遗症家庭病床及康复病区的患者按建床顺序单双数随机分成两组,各60例。单数为对照组,双数为观察组。两组均给予辨证施护和药物治疗、针刺、按摩和功能训练,观察组同时运用Orem自护理论进行护理,采用Barthel指数评定两组患者日常生活能力(ADL),观察两组患者护理并发症、护理意外发生情况及院内感染发生率。结果观察组克服心理障碍方法、接收信息途径、合理膳食与对照组比较,差异有统计学意义(P<0.01);护理并发症、护理意外及院内感染发生率较对照组均显著下降(P<0.05);观察组Barthel指数较对照组显著提高,差异有统计学意义(P<0.05)。结论 Orem自护理论应用于脑卒中后遗症患者辨证施护能促进患者各种功能的恢复,预防肌肉萎缩,关节粘连变形,促进神经代谢,提高护理质量,改善患者的ADL。

The Effect of Dialectical Nursing According to the Orem Self-Nursing Theory Applied to Patients with Sequelae of Stroke

Objective To observe the effect of nursing intervention using the Orem self-nursing theory on patients with sequelae of stroke.Methods 120 patients with sequelae of stroke in the rehabilitation ward and the family bed were randomized into two groups in order of building beds,all 60 cases,the odd numbers for the control group,the even numbers for the observation group.The two groups were given the dialectical nursing,medicine therapy,acupuncture,massage and functional training.Using the Orem self-nursing theory in the observation group at the same time.The two groups were assessed by Barthel index of activities of daily living(ADL).Care complications,nursing accidents and the incidence of nosocomial infection were observed in two groups.Results Methods to overcome the psychological barrier,ways to receive information,a reasonable diet beteween the observation group and the control group were significantly different(P<0.01).Care complications,nursing accidents and the incidence of nosocomial infection were significantly decreased in the observation group(P<0.05).Barthel index of ADL for the observation group was significantly improved compared with the control group(P<0.05).Conclusion The Orem self-nursing theory applied to patients with sequelae of stroke can promote functional recovery,prevent the muscle atrophy,prevent the joint adhesion and deformation,promote nerve metabolism,improve quality of care,improve patients′ ADL.
