| 摘 要： | 为了探讨偏执型精神分裂症血浆高香草酸（ｐＨＶＡ）与氟哌啶醇临床疗效的关系，对３５例偏执型精神分裂症病人以氟哌啶醇治疗６周，测定ｐＨＶＡ含量和药物浓度，并采用简明精神病评定量表、阳性症状评定量表、阴性症状评定量表进行临床评定。结果显示，与３８名正常对照者比较，全组病人及有效者的ｐＨＶＡ基础水平显著增高，治疗后ｐＨＶＡ逐步下降，阳性、阴性症状显著改善。无效者的ｐＨＶＡ基础水平与对照组无显著性差异，治疗后不下降，症状轻微改善。有效者与无效者在年龄、病期、症状严重程度及血药浓度方面无显著性差异。提示偏执型精神分裂症可能存在两种多巴胺功能亚型

| Abstract: | Toexploretherelationbetweentheplasmahomovanilicacidandtheresponsetohaloperidoltreatmentinparanoidschizophrenics,35patientswithparanoidschizophreniaenteredthestudy.Plasmaho-movanilicacid(pHVA)anddruglevelsweremeasuredatbaselineandduring6weeksofhaloperidoltreatment.PsychopathologywasweeklyevaluatedwiththeBriefPsychoticRatingScale(BPRS),theScaleforAssess-mentofPositiveSymptoms(SAPS)andtheScaleforAssessmentofNeg-ativeSymptoms(SANS).38healthycontrolswereusedforcomparisonofbaselinepHVAlevels.With-inalpatientsandthegoodresponsers,thepHVAlevelwassignificantlyhigheratbaselinethaninthecontrols,anddecreasedgradualyduringthehaloperidoltreatmentwithasignificantimprovementinbothpositiveandnegativesymptoms,whileinthepoorresponsers,thepHVAlevelatbaselinewasal-mostequaltothatinthecontrolsandnotdecreaseddur-ingthetreatment,withaslightimprovementinpositiveandnegativesymptoms.Goodresponsersandpoorre-sponsersdidnotdifersignificantlyintermsofage,durationofilness,severityofpresentingsymptoms,ordrugconcentration.Theseresultssuggestthattheremayexisttwodopaminergicsubgroupsofparanoidschizophrenia. |
