摘    要:目的:对18F脱氧葡萄糖(18F-FDG)PET显像在监测卵巢癌复发中的临床价值进行探讨。方法:对22例怀疑复发的卵巢癌患者,行全身18F-FDGPET检查24例次,并与近期B超、CT、MRI检查结果相比较。复发诊断根据病理、血CA125监测及随访结果确诊。结果:24例次的PET检查中,阳性13例次,阴性11例次,阳性者证实全部为复发,阴性者中,1例CA125升高,后经随访确诊为复发。PET监测复发的准确性、灵敏度、特异性分别为95.7%、100%、91.7%。而B超、CT、MRI检查的准确性分别为70%、85.7%、44.4%。结论:PET较其他影像学检查能更早并准确发现可疑病灶,使患者获得早发现、早治疗的机会,是监测卵巢癌复发极有价值的方法。

Abstract:Objective:To evaluate the practice usefulness of PET imagining for detecting disease in patients with recurrent ovarian cancer.Methods:Twenty-three whole-body 18F-FDG PET studies were performed in 22 patients with suspected ovarian cancer.At the same time,the performance of traditional radiologic imaging(US,CT,MRI)and Ca125 measurement were used.PET results were compared with the results of conventional imaging,which related to pathological findings,Ca125 levels and clinical follow-up.Results:PET examination showed a specificity of 95.7%,a sensitivity of 100%,an accuracy of 91.7% for the diagnosis of recurrent ovarian cancer during 6~85 months among 22 cases who had previously received operation and chemotherapy.The coincidence between the results of B-ultrasound,CT,MRI examinations and pathologic finding were 70%,85.7% and 44.4%,respectively.Conclusion:18F-FDG PET may be more accurate and ealier to detect recurrence of ovarian cancer with negative findings of traditional imaging.It is a clinically valuable for post-operational follow-up of ovarian cancer in confirming the recurrent sites early and correctly.
