三维动态增强磁共振血管成像对门静脉高压症侧支循环的显示价值

摘    要:目的探讨三维动态增强MR血管成像(3D DCE MRA)对门静脉高压症患者侧支循环的显示情况及最佳时间。方法 38例门静脉高压症患者进行3D DCE MRA扫描,在动脉期扫描后25 s、50 s、75 s、100 s再重复扫描4次,对多次采集减影后的图像进行最大强度投影(MIP)重建,观察门静脉系统血管主干及侧支的显示情况,并统计患者所显示侧支血管的最佳时间。结果 34例门静脉主干与多数侧支循环血管显示清楚,4例血管显示模糊;门静脉主干与多数侧支循环绝大部分为25 s时相显示最佳,个别侧支血管显影延迟。结论呼吸控制及减影后MIP重建处理能提高对门静脉高压症患者侧支血管显示的清晰度,增加采集范围和重复动态采集可以减少侧支循环遗漏。

Abstract:Objective To investigate the display condition and best time of 3D dynamic contrast enhanced MR angiography(3D-DCE-MRA) for collateral circulation in portal hypertension. Methods 38 cases with portal hypertension were underwent 3D-DCE-MRA.Repeated scanned at 25 s、50 s、75 s and 100 s after scanned in arterial phase.The images which had been collected many times and subtracted were MIP reconstructed.Then observe the display of primary and collateral circulation of portal veins and count the best display occasion. Results The primary and most of the collateral venous circulation of 34 cases displayed clearly,while the other 4 cases displayed indistinctly.The best time phase was 25 s and the display of several collateral circulations were delayed. Conclusion The display of collateral circulation of the patients with portal hypertension is much clearer when use respiration control and MIP reconstruction.And increase image collecting areas and repeatedly and dynamically collect can decrease collateral circulation miss.
