晚发支气管哮喘与老年COPD气道反应性及肺功能特点

目的:探讨晚发支气管哮喘与老年COPD患者气道反应性等呼吸功能特点。方法:36例晚发支气管哮喘与58例老年COPD行通气功能,气道阻力,肺气肿分度及气道反应性比较。结果:缓解期晚发支气管哮喘97%存在气道高反应性(BHR),而COPD组BHR仅为12%,且乙酰甲胆碱浓度为哮喘组的4·8倍(P<0·005),COPD组V25下降达100%,且气道阻力在激发前后无明显改变,肺气肿分度以中重度为主。结论:COPD组小气道损害及肺气肿程度较重,而气道高反应性测定对于老年哮喘患者检出较为敏感,并对差异的可能机理进行了探讨。

The Charnctertzation of Bronchial Responsiveness and Pulmonary Functions in Elderly Patients with Late Asthma and COPD

Objective:To detect the characterization of Bronchial Reactivity and other pulmonary functions in elderly patients with late asthma and COPD.Method:Complicated the ventilative function,Raw,the degrees of pulmonary enphysemea and Bronchial Reactivity in both groups.Results:BHR in aged Asthma is 97% and in COPD is 12%.The concentration of Mch in COPD is 4.8 times higher than in aged As thma.V_ 25 decrease are 100% in group and the Raw are not changed after challenge test.The degrees of pulmonary enphysema are much severe than in aged as thma.Conclusions:The destruction of little airway and the degree of pulmonary enphysema are much severe in COPD.The Test of Bronchial Reactivity is more sensitive to asthma.
