后腹腔镜肾上腺嗜铬细胞瘤切除术与开腹手术的血流动力学比较

目的比较传统的开腹手术切除肾上腺嗜铬细胞瘤和后腹腔镜下嗜铬细胞瘤切除术对血流动力学的影响。方法选择44例单侧肾上腺嗜铬细胞瘤患者随机分为两组，腹腔镜组22例，侧卧位，经腹膜后途径的腹腔镜下行肾上腺嗜铬细胞瘤切除。开腹组22例，行传统的外科肾上腺嗜铬细胞瘤切除术。观察记录术中出现高血压的次数、低血压的次数、血压波动的幅度、升压药物的使用情况、手术时间、麻醉时间。结果手术时间两组间无统计学意义，出现术中高血压的比例腹腔镜组比开腹组低，肿瘤切除后出现术中低血压的比例两组之间无统计学意义，腹腔镜组的术后疼痛程度较轻。结论腹腔镜组的术中血流动力学较传统外科手术平稳。

Comparison of haemodynamics of retroperitoneoscopic treatment of Pheochromocytomas and surgical resection

Objective To compare the traditional open operation resection of adrenal pheochromocytoma and posterior laparoscopie adrenalectomy for pheochromocytoma effects on hemodynamics. Methods 44 patients of pheochromocytomas were randomly allocated onto 2 groups ,22 patents in the retroperitoneoscopic treatment group were treated with retroperitoneoscopic resection of pheochromocytoma,22 patients in the surgical group were treated with classic resection of pheochromocytoma. Hypertention episode, hypotension episode, range of blood change, vasoactive medicine were recorded. Results There was no difference in surgical time between the 2 groups, hypertension in the retroperitoneoscopic treatment group was less the that in the surgical group, there were no difference in hypotension after resection, less pain in the retroperitoneoscopic group. Conclusion The hemodynamics of retroperitoneoseopic treatment group was more stable than that in the surgical group.
