| 摘 要： | 目的：为克服传统胆肠吻合术治疗肝胆管结石存在的诸多不足，设计了一种新术式-皮下通道型胆囊肝胆管成形术。方法：在清除结石，切开肝门部胆管狭窄环，并做好肝门整形后，将胆囊壶腹部切开相应大小的切口与之吻合，并适当游离胆囊，使其底部可以被固定于切口皮下。结果：完成此术式87例，所有患者手术顺利，术后恢复良好，未发生并发症。结论：该术式既能有效地处理结石，纠正狭窄，又能保存胆囊，胆管及Oddi's括约肌的作用，保存胃肠道的正常生理通路及功能。

| Abstract: | Objective:To overcome the faults of cholangio jejunostomy,a novel operation was designed.Methods:Hartmann's pouch of gallbladder was used to anastomose stone strictured intrahepatic bile duct while leaving the common bile duct and Oddi's sphincter intact and fixing the fundus of bladder subcutaneously.Results:87 patients underwent the operation successfully,and recovered well. There was no postoperative complications.Conclusion:This operation not only keeps the normal physical function of gallbladder,hepatic bile duct,GI tract and Oddi's sphincter intact but also provides a new way to treat postoperative recurrence or remaining stone |
