CT引导三维立体定位经皮肝穿刺微波消融治疗肝癌的临床研究

目的 探讨应用CT引导经皮肝穿刺微波消融治疗肝癌的微创技术并评价其治疗效果及应用价值。方法 应用国产ECO-100经循环微波刀，经CT定位引导后在基础＋连续硬脊膜外麻醉下将微波针头，经皮肝穿刺进入瘤体进行微波消融毁损。结果 72例89个肝癌瘤体进行CT引导下经皮肝穿刺微波消融治疗（PMCT），全部瘤体原位毁损，随访8例复发，其中4例于原肿瘤区边缘复发，3例肝内其他部位复发，1例肺部转移，局部复发率为4．49％，所有PMCT患者未出现明显的副作用和其他并发症。结论 CT引导下三维立体定位经皮肝穿刺微波消融治疗肝癌作为肿瘤热治疗的一种方法，具有微创、时间短、安全方便、疗效确切、定位精确、重复性强等特点，具有良好的临床实用价值。

Clinical research of percutaneous microwave ablation treatment CT guided for liver cancer

Objective To explore the micro invasive technique to treat the liver cancer by using percutaneous microwave ablation under the CT guidance and evaluate the clinical effect and value. Methods We used percutaneous microware cool - tip thermotherapy system Model ECO - 100 which made in china. Under basic and epidural anesthesia, a pereutaneous microware needle was introduced with CT guidance into the tumour of the liver for microwave ablation. Results 72 patients with 89 liver cancer nodules were treated with percutaneous microwave coagulation therapy（PMCT） , All cancer nodules were ablated. 8 patients suffered local recurrence in liver and the sites of tumour were in the edge of primary tumour in 4 patients. 1 patient suffered distant recurrence in lung. The local recurrence rate was 4. 49%. There were no serious clinical side effects or complications in the PMCT patients. Conclusion PMCT is a kind of clinical way to liver cancer. It is micro invasive, safe, convenient and effective.
