急诊手术与择期手术治疗股骨颈骨折远期疗效比较

目的：探讨急诊手术与择期手术治疗股骨颈骨折的远期效果。方法：１２１例均以传统的闭式三翼钉内固定术式固定。伤后７２ｈ以内急诊手术５８例，胫骨结节牵引１～２周后择期手术６３例。从复位固定、住院时间、并发症、Ｘ线片及髋关节功能几方面进行比较分析。结果：急诊手术组不愈合２例（３．４％），股骨头缺血坏死３例（５．２％），髋关节功能优良率８６．２％，平均住院日２１ｄ。择期手术组不愈合１３例（２０．７％），股骨头缺血坏死１５例（２３．８％），髋关节功能优良率６３．５％，并发症９例（１４．３％），平均住院日４６ｄ。结论：股骨颈骨折的治疗效果与其接受手术治疗时间有关，手术愈早疗效愈好。

A Comparison of Long term Results between Emergency and Delayed Operation for Treatment of Femoral Neck Fracture.

Objective:To discuss the long term results of treating femoral neck fracture by emergency operation or selecting time operation.Methods:Traditional closed trinfin nail internal fixation was used in all the 121 cases.58 cases received emergency operation within 72 hours after injury.63 cases received selecting time operation after their tibia nodes had been pulled for one or two weeks.We compared and analyzed by several ways,such as reduction fixation ,in hospital time,complication,X ray photograph and the function of hip jont.Results:In the emergency operation group,non union of the femoral neck were two cases(3 4%).The ischemic necrosis of femoral head were three cases(5 2%).The fine rate of hip joint function recovery was 86 2%.The average in hospital time were 21 days. In the selecting time operation group:non-union of the femoral neck femoral were 13 cases(20 7%). The ischemic necrosis of femoral head were 15 cases (23 8% ). The fine rate of hip joint function was 63 5%.9 cases had complication.Average in hospital time were 46 days.Conclusions:The treating effect for femoral neck fracture is related to the time of operation.The earlier the operation is performed the better result is promised.
