薄层CT在鼻部外伤中的诊断价值

摘    要:目的:对比X线平片与薄层CT扫描对鼻骨骨折的诊断价值。方法:回顾性分析131例鼻部外伤患者的薄层CT检查与鼻骨侧位X线平片对鼻骨骨折的诊断率。结果:131例鼻部外伤患者鼻骨侧位X线平片诊断鼻骨骨折48例(36.6%),可疑骨折22例(16.8%),未见骨折61例(46.6%);薄层CT检查显示:鼻骨骨折83例(63.4%),可疑骨折3例(2.3%),未见骨折45例(34.3%)。薄层CT诊断鼻骨骨折与X线平片差异有非常显著性(P<0.01)。结论:与X线平片相比,薄层CT可更清晰地显示鼻骨骨折错位的程度及对鼻部邻近结构如鼻中隔、筛骨、上颌骨损伤的显示则更为优越,临床疑有鼻面部骨折者应首选薄层CT检查。

Abstract:Objective To compare The capability of X-ray plain film VS Thin slice CT scan for diagnosing the nasal fractures.Methods The diagnostic rate of thin slice CT sean for nasal fractures was retrospectinely analyzed in 131 patients with nasal trauma and compared with that of lateral X-ray plain films.Results Among 131 patients with nasal trauma,lateral X-ray plain films of nasal bones showed nasal fractures in 48(36.6%) cases,Suspected fractures in 22(16.8%)cases,and non-fracture in 61(46.6%)cases while thin slice CT sean showed nasal fractures in 83(63.4%)cases,suspected fractures in 3(2.3%)cases,and non-fracture in 45(34.3%)so that the diagnostic rate between thin slice CT scan and X-ray plain film for nasal fracture was significant difference (P<0.01).Conclusion Compared with X-ray plainfilm,thin slice CT sean can more clearly reveal the displacement degrees of nasal fractures and nasal neighbor structures,such as the injuries occurred in nasal scptum,ethmoid bone and superior maxillary bone and therfore anybody who with clinically suspected naso-facial fracture should first select thin slice CT esamination.
