原发性小肠肿瘤88例临床分析

目的 提高对原发性小肠肿瘤的认识及诊治水平.方法 回顾性分析2000年1月至2010年6月收治的88例原发性小肠肿瘤患者的临床资料.结果 88例患者中,良性肿瘤占22.7%(20/88),恶性肿瘤占77.3%(68/88).良性肿瘤以平滑肌瘤多见,占60.0%(12/20);恶性肿瘤主要为腺癌,占45.6%(31/68).88例患者主要临床表现依次为腹痛及腹部不适、消化道出血、体重减轻及消瘦、贫血、腹部包块等.本组术前误诊率为62.5%(55/88),全组均行手术治疗,无手术死亡病例.恶性肿瘤1、3、5年生存率分别为74%(37/50)、46.0%(23/50)、24.0%(12/50).结论 小肠肿瘤临床表现不典型,早期诊断困难,误诊率高,一经诊断,手术治疗是最佳选择.

Clinical analysis of 88 cases of primary small intestine tumor

Objective: To explore the diagnosis and treatment of primary small intestine tumor.Methods: Retrospective analysis on clinical data of 88 cases of primary small bowel tumors from January 2000 to June 2010.Results: In the 88 cases,the benign tumor were 22.7%(20/88),while the remaining 77.3%(68/88) were malignant.The former were mostly leiomyoma,accounting for 60.0%(12/20),adenocarcinoma was the most common type of malignant(45.6%,31/68).Out of the 88 cases,the main clinical manifestations were abdominal pain and abdominal discomfort,gastrointestinal bleeding,weight loss and weight loss,anemia,abdominal mass.The preoperative misdiagnosis rate was 62.5%(55/88),the whole group underwent surgical treatment,no operative deaths,the 1,3,5,year survial rates of malignant were 74%(37/50),46.0%(23/50),24.0%(12/50),respectively.Conclusions: It is difficult to make an early diagnosis of primary small intestine tumor because of its atypical clinical manifestations.The misdiagnosis rate is high,once diagnosed,surgery is the best option.
