脂联素与2型糖尿病大血管并发症的关系探讨

目的:探讨脂联素(APN)与2型糖尿病及其大血管并发症的关系。方法:分别测定正常健康对照组(NC组,20例),单纯2型糖尿病组(T2DM组,46例),2型糖尿病合并大血管病变组(T2DM+MA组,54例)血清APN、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、糖化血红蛋白(HbA1c)、空腹血糖(FPG)、体重指数(BMI)、腰臀比(WHR)等。结果:NC组、T2DM组及T2DM+MA组血清APN依次降低,且差异有统计学意义(P<0.05);血清APN水平与BMI、WHR、TC、HbA1c呈显著负相关;多元逐步回归分析表明WHR、TC成为影响脂联素水平的最为显著的因素。结论:低血清APN可能与T2DM及其大血管并发症的发生发展有关,APN在糖尿病动脉粥样硬化过程中起重要作用。

Study of relationship between serum adiponectin and macrovascular complications in type 2 diabetes mellitus

Objective:To investigate the changes of serum adiponectin concentrations in different stages(with or without macrovascular complications) of T2DM,and to explore the correlation of adiponectin with T2DM and its macrovascular complications.Methods:The levels of serum adiponectin,triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),HbA1c and fasting plasm glucose(FPG),body mass index(BMI),waist hip ratio(WHR) were measured in normal subjects,type 2 diabetic patients and type 2 diabetic patients with macrovascular complications.Results:The serum adiponectin level was significantly lower in type 2 diabetic patients than that in normal subjects,and was the lowest in type 2 diabetic patients with macrovascular complications.Serum adiponetin level was negatively correlated with BMI,WHR,TC,HbA1c.Mutiple regression analysis showed that WHR and TC independently affected the level of serum adiponection.Conclusion: Hypoadiponectinemia is related to the development of T2DM and T2DM with macrovascular complications and maybe plays an important role in atherosclerosis in type 2 diabetic patients.
