经血液与经皮肤测胆红素浓度两种方法的比较

用经皮肤测胆红素仪(TcBM)测新生儿黄疸读数的同时抽血检测血清胆红素浓度,发现两者之间存在直线相关,其变异系数接近,表明TcBM可用作新生儿黄疸的筛选工具;若将黄疸按病因及光疗与否分组,建立各组的TcBM读数与血清胆红素浓度间的回归线,则TcBM可用来监测黄疸的变化。

COMPARISON OF TWO METHODS FOR DETERMINATION OF BILIRUBIN LEVELS FROM SERUM AND BY TcBM

A total of 278 transcutaneous bilirubinmeasurements (TcBM) were performed in112 Chinese newborns with jaundice: 174data were obtained from those not receiv-ing phototherapy, 104 data from those re-ceiving phototherapy. When TcBM readings were taken, bloodsamples from the same patients were obtain-ed for comparison with serum bilirubinconcentration by using different traditionalmethods and three different kinds of in-struments (Model 721, 72 spectrophotometerand Bilirubin Meter Model-333, OpticalWorks, LTD). We observed that a linear relationshipexisted between TcBM readings and the serumbilirubin levels. The coefficient of variationof various blood samples of the same TcBMreading and the coefficients of variation ofconcentration of any one blood sample fromthree different kinds of instrument werecalculated separately based on statistics.When the newborns were divided intogroups according to the cause for jaundicethe ranges of coefficients of variation of bi-lirubin obtained through TcBM readingsand traditional methods were very close.This result indicates that TcBM can be usedas a screening device in neonatal jaundice,and it would permit one to monitor thenewborn bilirubin concentration (increas-ing or decreasing) within any group onc?the regressive linear equation of serumbilirubin concentration vs TcBM readingbeing established.
