右美托咪定对学龄前小儿眼科手术麻醉恢复质量的影响

目的:观察右美托咪定对学龄前小儿眼科手术麻醉复苏期咳嗽和躁动的预防作用.方法:小儿眼科手术30例,随机分为2组,每组15例.两组麻醉诱导方案相同,B组术中靶控丙泊酚,A组诱导前静注右美托咪定(初始量0.6 μg·kg-1,输注10 min),术中靶控丙泊酚同时右美托咪定0.5 μg·kg-1·h-1静脉维持,术前10 min停止麻醉药物输注.记录麻醉苏醒期咳嗽的次数和程度、躁动发生情况以及拔管时间.结果:B组中麻醉复苏期间咳嗽的发生率、程度和躁动发生率均显著高于A组(P<0.01),两组拔管时间差异无统计学意义(11.9±2.5 min vs 12.1±3.1 min,P>0.05).结论:学龄前小儿眼科术中应用右美托咪定能够减少麻醉复苏期间咳嗽、躁动的发生率,提高麻醉恢复质量.

Influences of Dexmedetomidine on Anesthetic Recovery Quality in Preschool Children with Ophthalomologic Surgery

Objective: To investigate the preventive effect of dexmedetomidine on coughing and agitation during the recovery period in preschool children with ophthalomologic surgery.Method: 30 preschool children undergoing ophthalomologic surgery were enrolled and randomly allocated into two groups: group A(propofol combined with dexmedetomidine,n=15) and group B(propofol,n=15).In the group B,propofol was administered under target-controlled infusion during the operation.In the group A,dexmedetomidine(with the initial dose of 0.6 μg·kg-1 in 10 min)was given before the induction,and during the operation process,propofol was administered under target-controlled infusion combined with 0.5 μg·kg-1·h-1 dexmedetomidine continuous infusion,and the infusion of dexmedetomidine was terminated 10 min before the beginning of the surgery.The number of coughing,extent of agitation and time of extubation were observed.Result: In comparison with the group A,higher rate of coughing,the coughing extent and agitation in the group B were shown(P<0.01) while no statistical significance was observed in the time of extubation between the two groups(11.9±2.5 min vs 12.1±3.1 min,P>0.05).Conclusion: Dexmedetomidine can decrease the incidence of coughing and agitation,and enhance the anesthetic recovery quality in preschool children with ophthalomologic surgery.
