CT对肝内周围型胆管细胞癌的诊断价值

目的：探讨螺旋CT对周围型肝内胆管细胞癌（IHPCC）的诊断价值。方法：回顾性分析16例经病理证实周围型胆管细胞癌的常规CT平扫及动态增强扫描。结果：16例IHPCC中10例位于肝左叶。肿块型12例，CT平扫表现为低密度肿块，早期无强化（n=5）或边缘强化（n=7），延迟期内部强化。浸润狭窄型2例，表现为胆管壁局限性增厚伴强化，腔内息肉型1例，扩张胆管内乳头状强化结节，1例未见明确肿块。13例伴胆管不同程度扩张。结论：CT动态增强是IH—PCC的诊断有重要价值，早期无强化或边缘强化，延迟期病灶内部强化，伴病灶周围胆管扩张是IHPCC的重要征象。

The Value of Helical CT in Diagnosis of Intrahepatic Peripheral Cholangiocarcinoma

Objective:To evaluate the diagnostic value of helical CT for intrabepatic peripheral cholangiocarcinoma(IHPCC). Methods:Sixteen patients with CT examination and histological diagnosis of IHPCC were retrospectively reviewed.Results:10 of 16 IHPCCs were located at left hepatic lobe.Mass-type was found in 12 cases which depicted as low density focus.No con- trast enhancement(n=5)or rim-like enhancement(n=7)were seen in early phase,and enhancement of the center of tumor were showed in delay phase.Two cases with infiltrative-type manifested as focal thickened wall with enhancement,1 case with polypoid-type with a enhanced mass within the dilated bile duct,and 1 case without definite tumor.Segmental biliary ductal dilatation were seen in 13 cases.Conclusion:Dynamic CT is an useful imaging modality in the diagnosis of IHPCC,and delayed enhancement with biliary ductal dilatation was characteristic signs for IHPCC.
