延颈交界处肿瘤的临床特点及手术治疗(附39例报告)

目的探讨延颈交界处肿瘤的临床特点及手术治疗。方法回顾性分析手术治疗的延颈交界处肿瘤39例病人的临床资料，其中行经后正中入路手术35例，经远外侧入路4例。结果髓外肿瘤34例，以神经鞘瘤和脊膜瘤为主；髓内肿瘤5例。全切除33例，次全切除5例，部分切除1例。出院时好转及治愈34例，无变化3例，死亡2例。结论延颈交界处肿瘤以髓外良性肿瘤多见。根据病变特点选择合适的手术入路，尽量减少对延脊髓的牵拉，是提高疗效的关键。

Clinical features and surgery of tumors in the medullocervical conjunction: report of 39 cases

Objective To probe the clinical features and surgery of tumors in the medullocervical conjunction. Methods Thirty-nine cases of tumors in medullocervical conjunction treated by operation were retrospectively analyzed. Among them 35 cases underwent operation via posterior middle approach, and 4 via far lateral approach, Results Extramedullary tumors were found in 34 cases, mainly including neurinoma and meningioma, and intramedullary tumors in 5 cases. The tumor was totally resected in 33 cases, subtotally in 5, and partly in 1. On discharge time, 34 cases had good recovery, 3 were stable, and 2 dead. Conclusions Extramedullary benign tumors formed main part of the tumors in the medullocervical conjunction. It is important to improve the operative outcomes by choosing proper approach and reducing traction of the spinal cord.
