腰椎间盘突出再手术36例原因分析

探讨腰椎间盘突出症再手术原因。方法对３６例腰椎间盘突出症再和术的病例进行回顾分析。结果再手术原因主要为术前漏诊及术前定位错误，椎间盘切除不彻底、对神经根管狭窄认识不足、不注意瘢痕粘连的预防、椎间盘感染及血肿等。结论腰椎间盘突出症手术前诊断及定位需准确，髓核切除要干净，并松解神经根，必要时扩大神经根管。

Analysis of the causes of re-operation in lumbar intervertebral disc herniation

Objective To explore the causes of re operation in lumbar intervertebral disc herniation. Methods 36 cases of re operation who had diskectomy in index procedure were analysed retrospectively. Results The major causes of re operation included misdiagnosis and mis orientation, incomplete discetomy, unaware of stenosis of radicular canal, ignorant of preventing cohesion of the scar, infection of intervertebral disc and hemotoma. Conclusion Diagnosis and orientation must be accurate before operation and nucleus pulposus removed completely. Nerve root should be released completely. Radicular canal is magnified if necessary.
