面中部骨折复位固定的临床分析

目的：探讨面中部骨折的特点及其手术复位固定的方法。方法：我院收治的面中部骨折患者53例，术前均行螺旋CT三维重建，根据骨折类型确定手术方案，术中辅以牵引钉颌间咬合复位固定，应用微型钛板根据骨折的部位和形态行坚固内固定。结果：51例患者术后创口均一期愈合，面型良好，咬合关系恢复，开口度2指以上，骨折段愈合良好，仅2例伤口感染引流后愈合。结论：结合三维重建技术进行检查，术中颌间咬合固定，对面中部骨折行切开复位内固定是一种安全可行的手术方案。

Clinical analysis of reduction and fixation in midface fracture

Objective:To investigate the characteristics of midface fracture and the method of surgical restoration and fixation.Methods:Fifty-three cases of central fracture were investigated.Three dimensional reconstruction of spiral CT was performed before operation in all cases and the surgical plan was confirmed according to the type of fracture.In operation, auxiliary intermaxillary traction titanium screw was used to help restoration,besides the micro titanium plate to strengthen in-ternal fixation according to the position and shape of the fracture.Results:Fifty-one cases of surgical wound were healing by first intention,with a good profile and a recovery of occlusion of 2-finger-large-open.While only 2 cases caught an infection and recovered after drainage.Conclusion:Open reduction and internal fixation is a safe and feasible surgical procedure for the fracture in the middle of the face,when combined with three-dimensional reconstruction technique for examination,intrao-perative occlusion of the jaw fixation in operation.
