手部高压注射伤精细微创治疗

目的探讨手部高压注射伤的精细微创手术治疗的效果。方法对12例手部高压注射伤患者在显微镜下精细一期清创、减压、冲洗引流,二期行游离植皮或皮瓣修复。结果患者均获得随访,时间6~12个月。手部活动度根据TAM功能评定法评价:优3例,良7例,中2例。结论手部高压注射伤是一种严重的复合性损伤,早期切开减压、精细清创、有效引流、延期闭合创口是有效的治疗方法。

Microsurgical treatment experience of chemical liquid high pressure injection injury of hand

Objective To investigate the clinical effect of the high pressure injection injuries of the hand by minimally invasive treatment under microscopy. Methods Twelve patients with high pressure injection injury in hand were giv-en early surgical debridement,decompression,flushing drainage,then in the second phase, the skin graftings or flaps transplantation were used to close the wound. Results All patients received postoperative follow-up with 6 ~12 months. According to TAM evaluation method for the functional assessment:excellent in 3 cases,good in 7 cases,fair in 2 cases. Conclusions High pressure injection injury in hand is a serious compound injury,effect treatment should be with early incision decompression,thorough and careful debridement,valid drainage and defer closing wound.
