无创BiPAP通气治疗急性左心衰竭疗效观察

摘    要:目的观察经口鼻面罩双水平气道正压通气(BiPAP)治疗急性左心衰的临床疗效。方法我院2004~2006年10月收治的62例急性左心衰患者。采用病历对照研究,对照组(30例),接受抗心衰常规药物治疗;BiPAP治疗组(32例),常规药物联合应用BiPAP辅助通气治疗,在治疗第1、4h分别监测血气及血压、心率、呼吸频率等指标。结果BiPAP治疗组患者在第1h内即可迅速改善临床症状,其心率、呼吸频率和各血气指标较对照组明显得到改善。对照组在常规药物治疗第4h才能缓解临床症状。结论早期应用BiPAP通气治疗能快速缓解急性左心衰的症状。

Abstract:Objective To observe the clinical effects of bilevel positive airway pressure( BiPAP )mechanical ventilation by oral-nasal mask in the treatment of acute left heart failure. Methods To perform case-controlled study in 62 cases of acute left heart failure patients admitted by our hospital from 2004 to Oct 2006.Control group (n=30) was treated using general anti-heart failure drug. BiPAP group(n=32) was treated using BiPAP ventilation and general anti-heart failure drug. Heart rate, breath rate, blood gas and blood pressure were measured after treatment 1 hour and 4 hours, respectively. Results After BiPAP treatment for 1 hour, clinical symptoms could quickly improve and heart rate, breath rate and blood gas were also significantly improved,compared to control group. In control group, however, clinical symptoms could relive after general anti-heart failure drug treatment for 4 hours. Conclusion Early using BiPAP ventilation treatment had apparent clinical effects on acute left heart failure patients.
