静脉麻醉下内窥镜诊疗应用200例

目的　评价静脉麻醉下行内窥镜诊疗的效果和价值。方法　麻醉组 2 0 0例行胃肠镜检查患者行芬太尼和异丙酚静脉麻醉 ,肠镜受检者还应用病人自控镇静技术 ,使用含异丙酚的Grasby 93 0 0PCA镇痛泵 ,待起效后开始操作 ;2 0 0例非麻醉组 (对照组 )患者按常规操作方法行胃、肠镜检查。比较 2组胃镜检查过程中不适、肠镜检查平均进镜时间和检患者查成功率 ,观察检查后的反应。结果　麻醉组胃镜检查中均无不适 ,而对照组有 86.2 % (P <0 .0 1)。肠镜麻醉组缩短了肠镜进镜时间 (P <0 .0 5 ) ,但检查成功率无显著差异 (P >0 .0 5 )。检查后麻醉组的反应明显好于对照组 (P <0 .0 1)。结论　静脉麻醉下行内窥镜诊疗可以明显减少患者的痛苦 ,增加了患者的依从性 ,不良反应少 ,优于常规检查。

Endoscope under vein anaesthesia applied 200 patients

Objective To evaluate the effect and the value of endoscope under vein anaesthesia.Methods 200 patients of anaesthesia group undergoing gastroscope or colonoscopy were injected with fentanyl and prpofol,and the patient-controlled sedation applied to the patients on colonoscopy ,who could use prpofol Grasby 9300 PCA .When the drugs acted ,doctors began to operate the endoscope.The other 200 patients without anaesthesia(the control group )were operated according to routine . In two group of patients ,discomforts on gastroscope ,operation duration and success rate of colonoscopy were compared . Operation reaction after examination were recorded too.Results The 75 patients of anaesthesia group during gastroscope did not complain any discomfort,and the 75 patients (86.2%) of control group had many trouble(P<0.01).Colonoscopy Oeration duration of the anaesthesia group was less(P<0.05),but success rate of the two group had no difference (P>05). Operation reaction of the anaesthesia group after examination was much better than the control group(P<0.01).Conclusions Conclusion Obviously ,endoscope under vein anaesthesia could reduce the pain of patients,acquire the cooperation,and has little reaction .It is superior to the normal operation.
