摘    要:目的探讨外周血白细胞及其亚类与冠状动脉病变的关系。方法选择初次行冠状动脉造影术的冠心病患者,测定外周血白细胞及亚类。采用logistic回归模型分析白细胞及其亚类与冠状动脉病变的关系。结果共纳入340例患者,中位数年龄64岁,61.0%为男性。白细胞中位数为6.34×109/L(2.74~25.30)×109/L]。logistic回归模型分析发现淋巴细胞绝对数(OR=1.053,95%CI:1.010~1.132,P<0.05)和单核细胞绝对数(OR=6.553,95%CI:2.425~20.236,P<0.05)对狭窄程度大于或等于75%冠脉病变有预测作用;单核细胞绝对数(OR=8.35,95%CI:4.468~18.234,P<0.05)对多支血管病变有预测作用。结论单核细胞和淋巴细胞增高对冠状动脉粥样硬化严重程度有预测价值。

Abstract:Objective To investigate the relationship of peripheral white blood cell and its subtypes with severity of stenosis in coronary atherosclerotic heart disease.Methods 340 patients with coronary atherosclerotic heart disease referred for coronary angiography for the first time in our hospital were enrolled in this study.Total white blood cell count and its subgroups were measured on admission.We analyzed the relationship of WBC and its subtypes with stenosis severity by logistic regression.Results The median age was 64 years and 61.0% of the patients were men.The median of WBC count was 6.34 × 109 /Lrange(2.74-25.30)×109/L].The logistic regression model revealed that lymphocyte count(OR=1.053,95%CI:1.010~1.132,P<0.05),and monocyte count(OR=6.553,95%CI:2.425~ 20.236,P<0.05) were predictive of stenosis ≥75%;monocyte count(OR=8.35,95%CI:4.468~18.234,P<0.05) were predictive of the presence of multivessel disease.Conclusion In conclusion,Monocyte count and lymphocyte count are predictive of severity of coronary atherosclerosis.
