整体护理干预对子宫内膜癌患者术后生活质量的影响

目的探讨整体护理干预对子宫内膜癌患者术后生活质量的影响。方法选择解放军海军总医院2009年1月~2013年1月收治入院的子宫内膜癌患者200例,将其分为观察组与对照组,每组各100例。两组均常规进行药物治疗、常规化疗及常规护理。观察组在常规基础上从心理、用药、健康宣教、并发症几个方面对其进行整体护理。测试两组干预前、干预后焦虑自评量表（SAS）、抑郁自评量表（SDS）及癌症患者生活质量问卷（QLQ-C30）评分,并进行统计学分析。结果观察组干预前后,SAS、SDS量表分值差异均有统计学意义（P〈0.05）,对照组两次测查量表分值未见明显变化（P〉0.05）;观察组患者SAS、SDS量表分值与对照组比较,差异均有统计学意义（P〈0.05）;观察组干预后QLQ-C30量表结果显示,功能量表中的情感功能、认知功能、社会功能,整体健康状况以及症状量表中的疲乏、恶心呕吐、食欲不振等项的分值与干预前比较有明显改善,差异有统计学意义（P〈0.05或P〈0.01）;对照组患者第2次生活质量测查中,只有症状量表中的疼痛、呼吸困难两项分值与第1次测查结果比较差异有统计学意义（P〈0.05）。结论护理人员对子宫内膜癌患者进行整体护理,能有效缓解其焦虑情绪,提高其自尊感,改善其生活质量,对临床治疗起到了积极的作用。

The overall effect of nursing intervention on quality of life of patients with endometrial cancer

Objective To investigate the effect of nursing intervention on quality of life of patients with endometrial cancer. Methods 200 patients with endometrial carcinoma in Navy General Hospital of PLA in January 2009 - January 2013 admitted to hospital patients, were randomly divided into observation group and control group according to the principle with 100 cases in each group. Two groups underwent routine drug therapy, chemotherapy, nursing. The observation group in the conventional basis from the psychology, medicine, health education, complications of several aspects for the holistic nursing care. Two groups of the self rating anxiety scale （SAS）, self rating depression scale （SDS） and cancer patients quality of life questionnaire （QLQ-C30） score before intervention after the intervention, were tested and analyzed statistically. Results The observation group of SAS, SDS scale scores before and after the intervention showed significant difference （P 〈 0.05）, two measuring scale scores in the control group had no significant changes （P 〉 0.05）; compared with the control group, the patients in the observation group of SAS, SDS score had statistically significant differences （P 〈 0.05）; the observation group of QLQ-C30 function components showed that, in the emotional function, cognitive function, social function, overall health status scale of overall health, and symptom scales of fatigue, nausea and vomiting, loss of appetite, the comparison of scores before and after intervention bad significant difference （P 〈 0.05 or P 〈 0.01）; while the control group only symptom scale of pain, difficulty breathing scores had significant difference （P 〈 0.05）. Conclusion Nursing staff of holistic nursing care for patients with endometrial cancer, can effectively alleviate the anxiety, improve self-esteem, improve the quality of life, to actively apply for clinical treatment.
