骨科老年患者术后认知功能障碍的护理

目的:探讨骨科老年患者术后认知功能障碍的护理要点。方法:对我科收治的55例术后发生认知功能障碍的老年患者分别采取术前详细了解病史、积极治疗基础疾病、心理护理,术后严密观察病情、疼痛护理、营养支持及用药护理等护理措施,观察护理效果。结果:所有患者术后恢复良好,POCD精神症状在术后5~14 d消失,平均时间(8.9±2.8)d,均痊愈出院,住院时间(12.5±2.6)d。本组患者护理前LOTCA评分为(58.6±16.7)分,护理后为(82.7±23.5)分,差异有统计学意义(P<0.05)。结论:对骨科老年患者应预防性地做好围手术期护理,同时,对于已发生术后认知功能障碍的老年患者应积极进行神经内科检查及治疗,以促进患者认知功能早日恢复。

Objective To study the orthopaedic elderly patients postoperative cognitive dysfunction nursing key points.Methods I branch of the 55 cases were occurred after cognitive dysfunction of older patients take preoperative detailed understanding of history,respectively,positive for the basic diseases,psychological care,postoperative close observation of the illness,and pain nursing care,nutrition support and care nursing measures such as medicine,nursing effect observation.Results All patients postoperative recovery is good,POCD mental symptoms after 5 to 14 days disappear,the average time(8.9±2.8)days,were recovered hospital discharge,the length of time the(12.5±2.6)days.The patient care LOTCA before score of(58.6±16.7)points,nursing for(82.7±23.5)points,the difference was statistically significant(P<0.05).Conclusion The orthopaedic elderly patients should be preventive do perioperative care,meanwhile,has happened for postoperative cognitive dysfunction of older patients should actively for medical examination and treatment nerve,in order to promote the cognitive function in patients with will recover soon.
