乳腺导管瘘40例外科治疗体会

目的:探讨手术切除瘘管治疗乳腺导管瘘的效果。方法:2002年1月～2008年1月,我院对40例乳腺导管瘘进行手术治疗。瘘管连同周围部分腺体切除35例,3例行瘘管所在象限腺体切除,2例行半乳切除。34例一期缝合,6例敞开换药。结果:40例随访3个月～6年,平均3年,一期缝合的34例中有5例复发,经敞开换药后治愈,余35例均一次性治愈。结论:瘘管切除术是治疗乳腺导管瘘的基本手术原则,在减少复发的前提下,为保持乳房外形的美观及缩短住院时间,尽量予以一期缝合。

Surgical treatment for 40 cases of mammary duct fistula

Objective:To invetigate the effect of surgical treatment of mammary duct fistula.Methods: A total of 40 cases of mammary duct fistula were surgically treated,including 35 cases of fistuletomy and partial mastectomy,3 cases of Quarter Breast mastectomy,and 2 cases of Half Breast mastectomy.The incision was primarily sutured in 34 cases and kept open for drainage in 6 cases. Results: A follow-up was carried out in the 40 cases for 3 months～6 years (mean ,3 years ).Out of the 34 cases of primary closure of wound,5 cases developed a recurrence and then were cured by draiange and regular dressing change. The remaining 6 cases were cured on one session.Conclusion:Fistulectomy is the treatment of mammary duct fistula .For a good coametic apperance and a sgour hospital stay ,the operation should be performed with primay closure of wound ,unless the recurrence is more likely to happen.
