经桡动脉单导管相继完成选择性冠状动脉造影和肾动脉造影

摘    要:目的:探讨经桡动脉途径应用一条冠状动脉造影导管相继完成选择性冠状动脉造影和肾动脉造影的可行性和安全性。方法:42例临床诊断为冠心病伴高血压需行冠状动脉和肾动脉造影进一步明确诊断的患者,Seldinger法穿刺右桡动脉,应用5F Terumo左右共用型冠状动脉造影导管行选择性冠状动脉造影后,继之行左右肾动脉造影。结果:40例经右桡动脉途径顺利完成选择性冠状动脉造影,其中28例行选择性肾动脉造影,12例因造影导管不够长而行非选择性肾动脉造影,平均X线曝光时间和手术操作时间分别为7.3±3.1和23.4±9.6分钟;2例分别因桡动脉痉挛和右锁骨下动脉严重迂曲导致导管无法到位,后改行右股动脉途径完成选择性冠状动脉造影和肾动脉造影;全部病例均无血肿、假性动脉瘤等并发症发生。结论:经右桡动脉途径应用左右共用型冠状动脉造影导管相继完成选择性冠状动脉造影和肾动脉造影安全可行。

Abstract:Objective:To investigate the feasibility and safety of transradial approach for sequential selective coronary and renal angiography with with one catheter.Methods:Sequential selective coronary and renal angiography were carried out by transradial approach with a 5F Terumo universal catheter in 42 patients with coronary heart disease and hypertension. Results:By right radial approach 40 cases underwent selective coronary angiography and 28 of them underwent selective renal angiography successfully while the others were treated with unselective renal angiography for shortage of catheter.The average duration of X-ray fluoroscopy and the procedure was 7.3±3.1和23.4±9.6 minutes respectively and the success rate was 95%.2 cases failed because of right radial artery spasm and severe tutorsity of right subclavian artery and turned to right femoral artery approach.No complications occurred during and after the procedure.Conclusion:Transradial approach for sequential selective coronary and renal angiography with with one universal catheter is feasible and safe.
