家庭医师责任制社区护理模式干预对高龄高血压影响的研究

目的探讨家庭医师责任制社区护理模式干预对高龄高血压患者血压控制的影响。方法收集了社区高龄高血压患者84例,按照不同社区随机分成对照组和实验组,每组各42例,在高血压常规管理的基础上对实验组通过制订个体化的治疗计划加强健康教育干预,而对照组不予干预,6个月后比较两组的高血压知识知晓率以及坚持治疗、血压控制情况,并进行统计学分析。结果经家庭医师责任制社区护理模式干预后的实验组坚持治疗情况、高血压知识知晓率、血压控制情况优于对照组,P均<0.05,两组比较有统计学意义。结论通过家庭医师与护士共同制订个体化的治疗、健康教育等计划,由护士跟踪服务对社区高龄高血压患者进行干预,提高其治疗的依从性、知识知晓率和血压控制率具有重要意义。

Study on Nursing Mode of Responsibility System of Family Doctors Intervention on Aged Hypertension

Objective To assess the mode nursing mode responsibility system of family doctors in community intervention effects on elderly patients with high blood pressure. Method Collected 84 cases older patients of hypertension in community, according to different community randomly divided them into control group and experimental group, 42 cases in each group. Besides implementing routine management about high blood pressure, carrying out the individualized health education intervention in the experimental group, while the control group no intervention. To comparing the knowledge of hypertension awareness, adherence to medication rates, the situation of blood pressure control in two groups after six months, and then do statistic analysis. Results The responsibility system of family doctors mode of community nursing intervention, adherence to medication rates ,awareness of knowledge about high blood pressure, rates of blood pressure control is better than that of control group, P〈0.05, comparing the two groups have statistical significance. Conclusion Through the family doctors and nurses to jointly formulate treatment, individual health education programs, community intervention on senile patients with essential hypertension by nurses tracking service awareness of knowledge about high blood pressure, rates of blood pressure control, it is of great significance.
