43例烟雾病的临床分析

目的通过全脑血管造影(DSA)诊断及分析43例烟雾病(Moyamoya)病的临床表现和特征。方法分析北京天坛医院神经放射中心2009年2—9月经DSA确诊的43例Moyamoya病患者的临床资料及影像学资料。结果 43例患者年龄5～45岁,其中15岁以下11例,平均年龄27.5岁;脑卒中是本组Moyamoya病患者最常见的临床表现,发生率为76.7%(33例);儿童及青少年患者以缺血性脑卒中发病为主,而成年患者以出血性脑卒中发病为主;患者均有颈内动脉或其分支狭窄、闭塞和Moyamoya血管形成。结论对于无脑血管病危险因素的短暂性脑缺血发作(TIA)或脑卒中患者,尤其有卒中史或反复发作者则要考虑Moyamoya病的可能;DSA是诊断Moyamoya病和对其搭桥手术后进行疗效观察的最直接检查手段。

Anlysis on Clinical Manifestation Characteristics of 43 Moyamoya Disease Patients

Objective To anlysis the clinical manifestation characteristics of 43 Moyamoya disease patients.Methods Clinical data and imaging data of 43 patients with Moyamoya diease diagnosed by DSA were analyzed retrospectively.Results The average age of the 43 patients was 27.5 years old.Stroke episodes were the main clinical manifestations with the occurrence rates was 76.7%.All patients showed stenosis or occlusion of the internal carotid arteries or their branches and extensive abnormal basicranial vascular network called Moyamoya vascular,developed at the basal area of the brain.Conclusion It is suggested that there is a great possibility of occurrence of Moyamoya disease when recurrent stroke episodes or TIA occurred in young people.DSA is the reliable method of choice in diagnosis of Moyamoya disease.
