Survivin、P16INK4a在非小细胞肺癌中的表达及意义

目的:探讨新的凋亡抑制基因survivin在非小细胞肺癌(Non-small cell lung carcinoma,NSCLC)中的表达,及其与抑癌基因P16INK4a表达的相关性.方法:用免疫组化的方法检测survivin和P16INK4a在56例NSCLC组织中的表达,同时抽取12例非肺癌的肺部良生病变手术标本作为对照组.结果:NSCLC组织中survivin阳性率64.3%、明显高于肺良性病变组织(8.3%)(P＜0.01),P16INK4a在NSCLC组织和肺良性病变组织中的失表达率分别为51.8%、16.7%(P＜0.01).NSCLC组织中survivin阳性率和P16INK4a失表达率均与NSCLC的病理分级、临床分期和淋巴结转移密切相关(P＜0.01).Survivin在NSCLC组织中P16INK4a表达阳性和阴性病例,阳性率分别为40.8%(11/27)和86.2%(25/29)(P＜0.01),且表达呈负相关(r=-0.459,P＜0.01).结论:在NSCLC中survivin的过表达和P16INK4a的失表达提示二者可能通过抑制细胞凋亡和促进细胞增殖在非小细胞肺癌的发生发展中起协同作用,同时检测survivin、P16INK4a的表达状态对非小细胞肺癌的诊断和预后有一定的参考价值.

Expression of survivin,P16INK4a in non-small cell lung carcinoma and their significance

Objective:To study the expression of a novel apoptosis inhibitor gene,survivin,in non-small cell lung carcinoma (NSCLC),and its correlation to the expression of P16INK4a gene.Methods:The immunohistochemistry was used to assay the expression of surviving and P16INK4a in tissues of fifty-six cases of non-small cell lung carcinoma,with 12 specimens of benign lesions as the control.Results:Survivin was expressed in 36 out of 56(64.3%)cases of NSCLC,and its expression rate in benign pulmonary diseases was only 8.3%(P<0.01).The negative P16INK4a protein expression rates in NSCLC tissues and benign impaired tissues were 51.8% and 16.7% respectively.Overexpression of survivin and negative expressionof P16INK4a in NSCLC were strongly related to the tumor grade,clinical TNM stages and lymph node metatstasis(P<0.01).In P16INK4a positive expression group and negative expression group,the positive rates of survivin expression were 40.8%(11/27)and 86.2%(25/29)respectively.The difference was significant(P<0.01)and the expressions of survivin and P16INK4a showed a reuerse correlation(r=-0.459,P<0.01).Conclusion:Overexpression of survivin and negative expression of P16INK4a in NSCLC indicated survivin and P16INK4a may play cooperative role in the tumorigenesis and development of NSCLC through inhibiting cancer cell apoptosis and promoting proliferation.So the expressions of Survivin and P16INK4a can be used as references for the diagnosis and prognosis of NSCLC.
