血液流变学测定在老年医学中的应用

本文采用XN—5型自动计时直显比粘度和切变速率可调的玻璃毛细管式粘度计和自动计时细胞电泳仪,对30例健康老年人,32例老年高血压病,35例老年慢支并感染病人进行了全套血液流变学指标的观察。检查结果表明,健康老年人全血粘度(两种切变速率测定全血粘度)男性高于女性(P<0.05)。血球压积和血沉男性与女性比较也有显著增高(P<0.05)。老年高血压病人显示两种切变速率的全血粘度、血浆粘度、红细胞电泳时间,与健康老人比较有非常显著的差别(P<0.01)。老年慢支并感染病人,除血沉外,6项血液流变学检查非常明显高于健康老年人(P<0.01)。增高的原因可能与高血压、慢支并感染、红细胞老化、变形能力差及聚集性增高有关。纤维蛋白原增加可能是血浆粘度增高的有关因素。认为血液流变学测定的增高,促使产生微循环障碍,组织灌注减少是这些疾病发生、发展的重要机制之一。

The Application of Rheology Measurement to Geriatrics

Assessments of blood rheology were undertaken in 32 cases ( over 60 years of age ) with gero-hypertension and 35 with chronic gero-bronchitis and infection, The results of these assessments were compared with those obtained from 30 healthy old people ever age of 60 years, It was found that the whole blood viscosity (of two types of shear rate ) in aged males was higher than that in aged females (p<0.05).The hematocrit and erythrocyte sedimentation rate(ESR) was also obviously higher than those in aged females ( p<0.05 ) . The whole blood viscosity,plasma viscosity and eryththrocytes electrophoresis time significantly differed from those in the healthy aged ( p<0.01 ) . The blood rheology assessments of six clauses in the aged with chronic bronchitis and infection also elevalted evidently, except for erythrocyte sedimentation rate . This may be related to the hypertension , the chronic bronchitis and infection, the senility , poor deformability and increased agglutination of the red blood cells. The increase of fibrinogen contents may be a relevant factor leading to the increase of plasma viscosity. The increase of blood rheology measurement , the promotion of producing microcircular disturbance and the reduction of tissues perfusion, we think , are one of the most important mechanisms of the occurence and development of these diseases.
