超声介入无水酒精治疗肝门静脉癌栓的探讨

目的 探讨超声介入无水酒精治疗门静脉癌栓的疗效，方法 18例肝门静脉癌栓行超声引导注射我水酒精治疗，每周1－2次，连续5－7次为一疗程，注射结束后再随访6－12个月，结果 18例门静脉癌栓经超声引导门静脉穿刺注射无水酒精后，15例（83．3％）有不同程度改善，其中7例（38．9％）癌栓消失，8例（44．4％）表现为癌栓缩小或停止发展，2例（11．0％）左右支皆有癌栓者均无效，1例（6．0）因脑转移中断治疗。结论 选择细针于超声引导下进行肝静脉癌栓穿刺无水酒精治疗，可取得一定疗效，其主要的副作用是较明显疼疼，强调掌握穿刺要点与必要的止痛是本技术的关节。

Study of interventional ultrasound in treatment of tumor thrombus of portal vein with absolute alcohol

Objective To assess the curative effect of interventional ultrasound in treatment of the tumor thrombus of portal vein with absolute alcohol.Methods Absolute alcohol was injected into the tumor thrombus of portal vein guided by ultrsound in eighteen patients with liver tumors . The operation was carried out one or two times each week and the course of treatment involved five to seven time injections continuously. Then the patients were followed up for 6 12 months.Results Fifteen patients ( 83.8% ) were improved in different degrees after the treatment.Among them the tumor thrombus disappeared in seven patients ( 38.9% ). The tumor thrombus became smaller or stopped developing in eight patients ( 44.4% ). It was not effective in two patients ( 11.0% ) with tumor thrombus in both leftand right portal veins.One patient ( 6.0% ) could not continue to be injected as intracephalic metastasis.Conclusions It is of curative effect to inject absolute alcohol into the tumor thrombus of portal vein with fine needles guided by ultrasound. The main side effect is that the patient has an obvious pain during the operation. So it is the key points to master the puncture technique and give analgetic when necessary.
