甲状腺上下极淋巴引流与中央区淋巴结相关性研究

摘    要:目的探讨甲状腺上下极淋巴引流与中央区淋巴结之间的相关性。方法在行单侧甲状腺腺叶切除的病例中,分别在对侧甲状腺上下极注射亚甲蓝,观察中央区淋巴结蓝染情况。结果上极注射组23例中发现中央区淋巴结蓝染8例,蓝染率为34.78%(8/23);下极注射组23例中发现中央区淋巴结蓝染17例,蓝染率为73.91%(17/23),差异有统计学意义(P〉0.05)。结论甲状腺上下极的淋巴引流可能存在不同方向及路径,中央区淋巴结尚不能作为甲状腺不同部位肿瘤共同的前哨淋巴结归属区域。

Correlation between the lymph drainage of upperpole and lowerpole of thyroid and the central region lymph node

Abstract:Objective To investigate the correlation between upper/lower pole of the lymphatic drainage and the central region lymph node in thyroid.Methods During the operation of unilateral removal of the thyroid gland,methylene blue was injected in the contralateral upper or lower pole of thyroid.Blue dye of the central lymph nodes were observed.Results Central blue-stained lymph nodes were found in 8 cases of the total 23 cases which were injected in the upper pole.The blue dye rate was 34.78%(8/23).In contrast,Central blue-stained lymph nodes were found in 17 cases of the total 23 cases which were injected with methylene blue in the lower pole.The blue dye rate was 73.91%(17/23).The difference was statistically significant.Conclusion Different directions and paths maybe exist in the lymphatic drainage of the upper and lower pole of the thyroid gland.The central region cervical lymph nodes are not yet considered as the common sentinel lymph nodes drainage area of thyroid tumors in different poles.
