经皮微刨钢板内固定在治疗Pilon骨折中的临床应用

摘    要:目的探讨经皮微创钢板内固定治疗在Pilon骨折中的临床应用。方法将52例住院治疗的Pilon骨折患者随机分为对照组与观察组,给予25例对照组患者解剖型钢板固定治疗,27例观察组患者接受经皮微创钢板内固定治疗。比较两组患者手术时间、术中出血量、术后引流量、骨折愈合时间、踝关节功能及术后并发症。结果观察组患者术中出血量、术后引流量、骨折愈合时间显著少于对照组(p〈0.01);两组患者手术时间未见显著差异(P〉0.05);观察组与对照组踝关节功能优良率分别为85.2%和72.0%,差异具有统计学意义(P〈0.01);观察组术后并发症发生率显著低于对照组(7.4%vs528.0%,P〈0.01)。结论经皮微创钢板内固定治疗Pilon骨折,骨折愈合时间快,踝关节功能恢复理想且并发症少。

Abstract:Objective To explore the clinical application of minimally invasive percutaneous plate osteosynthesis in Pilon fracture.Methods Fifty-two cases with Pilon fracture were randomly divided into control group and experience group.25 patients undergoing anatomical plate fixation were selected control group and 27 subjects were given minimally invasive percutaneous plate osteosynthesis treatment.Operation time,intraoperative blood loss,the postoperative drainage volume,fracture healing time,ankle joint function and complications were compared between two groups.Results Intraoperative blood loss,the postoperative drainage volume and fracture healing time in experience group were much less than that in control group(P < 0.01);there were no significant difference between two groups(P > 0.05);the excellent rate of ankle joint function were 85.2% and 72.0% in two groups(P < 0.05);the rate of postoperative complications in experience group was much lower than that in control group(7.4% vs 28.0%,P < 0.01).Conclusion Minimally invasive percutaneous plate osteosynthesis in Pilon fracture can promote fracture healing and ankle joint function recovery with less postoperative complications.
