38例鼻腔鼻窦内翻性乳头状瘤鼻内镜下切除术回顾性分析

目的分析鼻内镜手术在鼻腔鼻窦内翻性乳头状瘤外科治疗中的应用。方法38例鼻腔鼻窦内翻性乳头状瘤患者,于鼻内镜下(或结合下鼻道开窗及犬齿窝穿刺术)行肿瘤切除术,观察手术效果。结果38例患者中,术后仅有4例出现局部复发,再次鼻内镜下局部清理后治愈。本术式可用于大多数比较局限的鼻腔鼻窦内翻性乳头状瘤的切除,具有损伤小,不遗留面部瘢痕的优点。结论对经严格选择的鼻腔鼻窦内翻性乳头状瘤病例,鼻内镜下可较准确地判断肿瘤的原发部位及其范围,能够最大程度的完整切除肿瘤。

A retrospective analysis on the inverting papilloma in nasal cavity and/or paranasal sinuses surgically removed under nasal endoscope among 38 cases

Objective To analyze the clinical feasibility of the nasal endoscopic surgery in the treatment of inverting papilloma occurred in the nasal cavity and/or paranasal sinuses.Methods In- cluded in this retrospective study were 38 cases with inverting papilloma occurred in the nasal cavity and/or paranasal sinues,all treated under the nasal endoscope with the tumors removed,or auxillarily combined with the procedures of fenestration in the inferior nasal meatus or puncturing in the canine fossa among partial patients.Then,observed among them was the therapeutic effect of the therapy on the tumors.Results Among the all 38 cases treated by this therapy,only 4 cases were seen with the lesion reoccurred locally,who were successfully cured by re-treatment with local operation under nasal endoscopy.This kind of surgical procedures could be appropriately used for the treatment of most inve- rting papilloma localized in the nasal cavity and/or paranasal sinuses,with such advantages as being minimally invasive to the normal organs and with no scarring process in the facial region.Conclusions For most of appropriately selected cases with inverting papilloma occurred in the nasal cavity and/or paranasal sinuses,the tumors can be completely removed at the highest level under nasal endoscope be- cause of such advantages as judging on the primary focus of the tumor and its involvement to the sur- roundings with accuracy.
