IgA肾病证候要素及其与血清分泌型IgA的相关性研究

目的:初步探讨IgA肾病患者证候要素特点及各证候要素积分与血清分泌型IgA(SIgA)水平的相关性。方法:整理2000—2010年核心期刊上明确提出IgA肾病辨证分型的文献,确立IgA肾病证候要素。收集符合纳入/排除标准的61例IgA肾病患者中医四诊资料,进行证候要素及对应积分的统计,并收集其血液标本,应用双抗体夹心酶标免疫分析法测定标本中SIgA水平;观察各证候要素积分与SIgA的相关性。结果:本虚证候要素有:阴虚、气虚和阳虚,以阴虚为主;标实证候要素中风热、血瘀、湿热较为常见;IgA肾病患者血清SIgA水平较正常对照者明显升高;SIgA与湿热积分呈负相关。结论:SIgA水平代表的是机体邪正盛衰的整体状态。

Relationship between the Level of Serum Secretion IgA(SIgA) and Syndrome Factors of IgA Nephrosis(IgAN)

Objective:To investigate the characteristics of syndrome factors in IgAN patients and the relationship between the level of SIgA and each syndrome score.Methods:Collecte and analyze documents published on core periodicals from 2000 to 2010,which had mentioned the IgAN syndrome differentiation,then determine its syndrome factors.61 IgAN patients’ four physical examinations and venous blood were collected.Then statistics their syndrome factors and syndrome scores.SIgA in the blood collected was detected by ELISA.Observe the relationship between the level of SIgA and each syndrome score.Results:The deficiency syndrome factors include yin-deficiency,qi-deficiency and yan-deficiency,amond them,yin-deficiency is the most one.Wind-heat,blood stasis and damp-heat are common in the sthenic syndrome factors.Serum SIgA level in IgAN patients is higher than healthy person.SIgA level has a negative correlation with damp-heat.Conclusion:The level of SIgA may represent our position of excess and weakness of pathogen and vital QI.
