促性腺激素释放激素激动剂在子宫内膜异位症合并不孕治疗中的应用

目的：观察促性腺激素释放激素激动剂（GnRH-a）在子宫内膜异位症（EM）合并不孕症治疗中的应用价值。方法：选取90例Ⅰ～Ⅳ期EM合并不孕症患者作为研究对象，随机分为观察组与对照组各45例，所有患者均给予腹腔镜手术治疗，术后分别给予醋酸曲普瑞林与孕三烯酮治疗，疗程均为3个月，随访观察1年，比较两组患者的妊娠、EM复发及不良反应情况。结果：观察组EM复发率明显低于对照组，妊娠率明显高于对照组（P〈0．01）。治疗期间，观察组出现面色潮红3例；对照组出现体重增加2例、痤疮1例。两组患者不良反应均较轻微，未影响持续治疗。结论：腹腔镜手术治疗后联合促性腺激素释放激素激动剂（GnRH—a）治疗子宫内膜异位症（EM）合并不孕症，可降低EM复发率、提高妊娠率，值得推广应用。

Gonadotropin-releasing Hormone Agonist in the Treatment of Endometriosis with Infertility

Objective：To observe the gonadotropin releasing hormone agonist （GnRH-a） in the combined treatment of infertility in endometriosis （EM） value. Methods： 90 patients were treated Ⅰ～Ⅳ period the EM with infertility patients as research subjects were randomly divided into observation group and control group,table 45 cases, all patients were treated with laparoscopic surgery, triptorelin acetate gestrinone treatment were given after the course of treatment was 3 months, 1 year follow-up observation, the two groups were compared pregnancy EM recurrence and adverse reactions. Results： EM recurrence rate of the observation group was significantly lower than the control group, the pregnancy rate was significantly higher than that in the control group （P~0. 01）. During treatment, the observation group three cases of facial flushing; control group had 2 cases of weight gain, acne one cases. The two groups of patients with adverse reactions were mild and did not affect the ongoing treatment. Conclusion. Laparoscopic surgery for joint gonadotropin-releasing hormone agonist （OnRH-a） treatment of endometriosis （EM） with infertility, can reduce the EM relapse rate and improve the pregnancy rate, should be widely applied.
