地佐辛复合丙泊酚在宫颈锥切术中的临床应用

目的探讨地佐辛复合丙泊酚用于宫颈锥切术的安全性和麻醉效果。方法将120例CINⅢ级预行宫颈锥切术的患者(ASAⅠ～Ⅱ级)随机分为3组,即地佐辛复合丙泊酚组(Ⅰ组,n=40)、芬太尼复合丙泊酚组(Ⅱ组,n=40)和单纯丙泊酚组(Ⅲ组,n=40)。行静脉全麻,必要时追加丙泊酚。测定三组患者麻醉前、注药后3 min、苏醒时的平均动脉压(MAP)、心率(HR)、呼吸频率(RR)、血氧饱和度(SpO2),并记录丙泊酚总用量、定向力恢复时间、唤醒时间、体动反应和术后1 h的VAS镇痛评分及不良反应。结果Ⅲ组定向力恢复时间和唤醒时间明显长于Ⅰ组、Ⅱ组,术中的体动反应及丙泊酚总用量也明显多于Ⅰ组、Ⅱ组(P<0.05);术后1 h的VAS评分Ⅰ组优于Ⅱ组、Ⅲ组(P<0.01)。结论地佐辛复合丙泊酚适用于宫颈锥切术麻醉,并且术后镇痛效果好、并发症少,值得临床推广应用。

Application of dezocine combined with propofol for cervical conization

Objective To investigate the safety and feasibility of dezocine combined with propofol intravenous anesthesia for cervical conization.Methods 120 patients(ASA Ⅰ~Ⅱ)with cervical conization were randomly divided into three groups:dezocine combined with propofol group(group Ⅰ,n=40),fentanyl combined with propofol group(group Ⅱ,n=40),propofol group(group Ⅲ,n=40).The patients were performed for intravenous anesthesia and added to propofol necessarily.The MAP,HR,RR,SpO2 were observed before anesthesia,3 min after anesthesia and after surgery,and the total amount of propofol,anesthesia orientation force recovery time,the wake-up time,body movement,VAS scores and adverse reactions were recorded.Results There were significant differences in anesthesia orientation force recovery time,wake-up time,body movement and the total amount of propofol between group Ⅲ and group Ⅰ/Ⅱ.The VAS scores of group Ⅰ(1 h after surgery)was better than those in group Ⅱ and group Ⅲ(P<0.01).Conclusion Dezocine combined with propofol for cervical conization can be used safely,it has good postoperative analgesia effect with fower complications,and it is suitable for clinical application.
