性早熟女孩血清骨钙素的改变及其相关因素分析

摘    要:目的 :了解性早熟女孩血清骨钙素 (BGP)的变化以及与其相关的因素。方法 :特发性真性性早熟女孩 5 9例 ,年龄为 2 .5 0~ 9.92岁 ,平均 7.2 7岁。其中选出与正常发育女孩组 (对照组 14人 )年龄及发育分期匹配的 2 8人进行 BGP对比研究 ,统计方法用两独立样本的 t检验 ;骨钙素的测定采用放射免疫分析法 ;BGP与其他激素之间的关系采用多因素的偏相关分析法(n=5 9)。结果 :性早熟组血清骨钙素 (BGP)的浓度为 4 7.2 6± 2 8.95 ug/ L明显高于对照组的 2 4 .14± 2 3.82 ug/ L,差别有显著性意义 (t=2 .5 0 7  P=0 .0 16 ) ;在控制了其它因素后 BGP分别与骨龄、硫酸脱氢表雄酮 (DHEA~S)呈现正相关 (P<0 .0 5 ) ,而与其他激素的相关性则无显著意义。结论 :女性进入青春发育期后 BGP升高 ;随着 BGP升高 ,成骨过程加速 ,骨龄增加 ;女性青春期生长突增开始可能主要依靠肾上腺皮质所分泌的雄激素 ,雄激素又可通过促进蛋白合成增加骨基质

Abstract:Objective:To probe into the difference of serum bone Gla-protein (BGP) concentration with central puberty precocious (CPP) and normal girl, and BGP correlating hormons in CPP girls Methods:Chosen 28 CPP girls from 59 CPP girls (aged 2 50-9 92 y, average 7 27 y), there was no significant difference in age and the stage of sexual development between 28 CPP girls and 14 normal girls Serum BGP was determined by RIA The data was analyzed by means of independent T-testand bivariate correlation Results:Serum BGP concentration of CPP girls (mean 47 26+28 95 μg/L) was higher significantly than normal girls (mean 24 14~23 82 μg/L)( t =2 507, P =0 016); Serum BGP showed significant obverse correlations with boneage and serum DHEAS, but no association with LH, FSH, E2, IGF-1 and IGFBP3 Conclusion:The serum BGP step up in puberty girl; By the serum BGP increasing,the velocity of bone formation and bone age accelerate The girl's grow peak occurred in puberty stage depend to androgen secreted by adrenal cortex and androgen can increase bone matrix by means of facilitating protein synthesis
