AMI患者体表心电图与肌酸激酶同工酶峰值关系的探讨

目的探讨急性心肌梗塞(AMI)患者体表心电图(BCG)的病理性Q波、ST段与血清肌酸激酶同工酶(CK- MB)峰值的关系。方法对137例AMI患者体表ECG与CK-MB峰值进行相关性分析。结果前壁、下壁AMI患者体表ECG的病理性Q波导联数、最大Q波幅度、Q波总幅度及前壁AMI患者ST段抬高的导联数与CK-MB蜂值有明显正相关(P<0．05)。但下壁AMI患者ST段抬高的导联数与CK-MB峰值无明显相关性(P>0．05)。结论前壁、下壁AMI患者体表ECG的病理性Q波改变及前壁AMI患者体表ECG的ST段改变与CK-MB峰值存在正相关,通过体表ECG的病理性Q波、ST段改交可预测梗塞范围,评价预后。

The relationship between electrocardiogram and creatine kinase isoenzyme MB peak value in patients with acute myocardial infarction

Objective To investigate the relationship among pathologic Q wave, ST segment of electrocardiogram (ECG) and serum creatine kinase isoenzyme MB (CK-MB) peak value in patients with acute myocardial infarction (AMI). Methods The data of ECG and CK -MB in 137 patients with AMI was analyzed by using a correlation analysis. Results There were significant positive correlations among the numbers of pathologic Q waves, the maximum extent of pathologic Q wave, the assembled extents of pathologic Q waves (in the anterior and posterior walls AMI), ST segment elevation and CK -MB peak value ( P <0.05). But there was no significant correlation between ST segment elevation and CK - MB peak value in inferior wall AMI ( P >0.05). Conclusion There was a significant correlation between changes in Q waves of ECG in patients with inferior wall AMI, changes in Q waves and ST segment elevation of ECG in patients with anterior wall AMI and CK-MB peak value. It can help to estimate the infarct size and prognosis.
